Ohio Department of Education has decided that effective July 1, 2017 parents will no longer be able to use reasons of conscience as a valid exemption for declining immunizations for children enrolled in ODE licensed centers like Precious Ones.

A medical statement is required for preschool students only.  School-Age students do not need medical forms, as they are required with the public and private school systems when registering for elementary school.

If your child is enrolled in preschool at POLC a medical statement is required by Ohio state law for attendance.  This medical statement is required within 30 days of the first day of attendance at POLC, and is required to be re-submitted annually (Ohio allows 13 months from the date of the original medical form submission in order to allow for insurance coverage). This annual medical form is usually done during the child’s yearly exam.

It is POLC policy that enrolling preschool students be immunized with MMR, DTaP, IPV, Hep B, Hep A, PCV, Hib, and Varicella vaccines.  POLC feels that this measure will protect our children and staff and prevent the spread of disease.
